The declaration covers passport data, travel route, health status, vaccination information, and potential contact with infectious diseases. The form can be submitted electronically or on paper within 7 days before border crossing. This applies to all official border checkpoints (airports, land borders, seaports). Medical documentation is not mandatory unless specifically required by Vietnam's Ministry of Health for specific epidemiological situations. Visa runs are also subject to this requirement.
